Online Reporting Form

This form is for students to submit information about experiences in academic settings that have made them feel unwelcome, excluded, or maginalized.  This form will help the Academic Inclusion Coordinator identify trends across the University and in specific Schools and Departments that may need to be addressed with training and education.   

Please note that the staff member receiving your submission is a mandatory reporter.  This means that she must report any instances of harassment or discrimination in violation of KU policy to the Office of Institutional Opportunity & Access.

Open Office Hours for Students:

Office Hours are available for students to meet with Amanda on the following dates and times.  These will take place in the Sabatini Multicultural Resource Center (SMRC) Room 102.

  • Tuesdays, 4:00 - 5:00 PM
  • Wednesdays, 9:30 - 10:30 AM
